چکیده

Congestive heart failure (CHF) is a major public health problem in developed countries.1–3 It is a significant burden to patients, healthcare providers, and society.2–4 Several hospital-based reports have documented that a high proportion of patients with CHF have normal left ventricular (LV) systolic function,5 and 4 different epidemiological investigations further confirmed that nearly half of CHF subjects in the community have normal LV systolic function.6–9 This condition is commonly referred to as diastolic heart failure (DHF). Hospital readmission rates for patients with DHF are similar to those for patients with systolic heart failure (SHF),10 and it is estimated that DHF accounts for $25% of the total cost of CHF, which is estimated at $15 to $40 billion annually.4,11 A distinction between DHF and SHF is important because DHF is associated with better long-term survival5 and because these 2 forms of heart failure require different therapeutic approaches.

New concepts in diastolic dysfunction and diastolic heart failure: Part I: diagnosis, prognosis, and measurements of diastolic function.

There is growing recognition that congestive heart failure (CHF) caused by a predominant abnormality in diastolic function (ie, diastolic heart failure) is both common and causes significant morbidity and mortality. However, there is continued controversy surrounding the definition of diastolic dysfunction and the diagnostic criteria for diastolic heart failure. As a result, clinical therapeuti...

Working Group Report. How to diagnose diastolic heart failure

Diastolic heart failure has emerged over the last 10 years as a separate clinical entity. Diastolic heart failure accounts for approximately one third of all heart failure cases, especially in an elderly population, and its natural history, with an annual mortality rate of 8%, is more benign than other forms of heart failure with an annual mortality of 19%. Because of its rising incidence in ag...

Heart failure with normal ejection fraction: new diagnostic criteria and pathophysiological advances.

Heart failure (HF) is a highly prevalent complex cardiovascular syndrome, and its clinical presentation is usually associated with ventricular dilatation, decreased contractility and reduced left ventricular ejection fraction (EF). However, in the past two decades studies have demonstrated that many patients with signs and symptoms of HF have normal EF (higher than 50%). The great challenge for...

New diagnostic and therapeutic possibilities for diastolic heart failure.

Despite the fact that up to half of all heart failure occurs in patients without evidence of systolic cardiac dysfunction, there are no universally accepted diagnostic markers and no approved therapies for heart failure with preserved ejection fraction (HFpEF). HFpEF, otherwise known as diastolic heart failure, has nearly the same grim prognosis as systolic heart failure, and diastolic heart fa...

measured in every patient with heart failure. The advent of tissue Doppler (and other echo-Doppler techniques) has made it easier and more practical to identify abnormalities in diastolic function. Nonetheless, precise and comprehensive assessment of diastolic function requires the use of invasive catheterization techniques. However, if noncardiac and other cardiac causes of heart failure are e...
